RIC Grades 3-8 ELA Assessments

 ELA 3 - 8 Information from Erie 1 BOCES RIC:

PLEASE READ and FORWARD these instructions to the District Personnel overseeing the Grades 3-8 ELA assessments.

Included in this mailing are your Grades 3-8 ELA answer sheets, test booklet labels (two sets have been printed for each grade level), the Testing Answer Sheet Rosters, and the L24 form.  We have printed these answer sheets and labels from the student data submission to the Data Warehouse.  For those students who did not receive a preprinted answer sheet, you will need to use an extra answer sheet, complete the L24 form, and make sure that student data is entered and uploaded to Level 1 of the Data Warehouse.  Please forward the answer sheets, labels, rosters, and the L24 form along with this note to the Principal(s) of your Grades 3-8 building(s). 

To all building Principals:

            Please review the following instructions for these answer sheets with teachers and students:

ü      Use a No. 2 pencil for all answers.

ü      Read and follow all directions carefully.

ü      Shade bubbles completely.

ü      To change an answer ……… PLEASE DO NOT ERASE!  (This applies to the teachers as well.)

§         Changed answers should be CROSSED OUT,

§         If you decide to select the crossed out answer, then circle that answer.

ü      DO NOT make any stray marks in the bubbled areas of the answer sheet.

ü     Teachers should check and make sure answer sheets are completely filled in before returning them to the WNYRIC.

 

            If a student does not have an answer sheet, please use one of the **EXTRA SHEET** documents and:

·        Completely fill in the shaded area at the top of the front side of the answer sheet with the student’s information and fill in the student’s name on the back where a line for student’s name has been provided.  Fill in the boxes at the bottom with the student’s ID on each page where the boxes have been provided.  If the student’s ID is not 9 digits, please zero fill preceding the Student ID.   

·        Write the name of the student on the Testing Answer Sheet Roster (class list) with their student ID.

·        Complete the L24 form and place this form and the extra answer sheets on top of all other answer sheets.  Please be sure to add this student’s information into your student management system and make sure it is uploaded into the Data Warehouse.

·        If a student’s name appears on the Testing Answer Sheet Roster but does not take the test, please write the reason why the student did not take the assessment on the roster IN ADDITION to bubbling under “Reasons Not Tested” on the answer sheet,  if applicable.  (Reasons Not Tested for ELA are:

Absent for entire test – when a student is absent for ALL parts of the test and the makeup dates.  NOTE:  If a student is absent for only a part of the test, please fill in the Absent bubble associated with that part of the test.  Administrative Error, No Score - if a student was excluded from testing due to an administrative error.  Medically Excused – if a student is too incapacitated by illness or injury to be tested. (Please contact your district CIO if further clarification is needed regarding Medically Excused.)

 

For all students taking the assessment:

 

 

Fill in the appropriate bubble(s) for IEP or 504 Plan Accommodations and/or LEP Accommodations, if applicable.

 

Fill in the Scoring Model Code, which is REQUIRED.

 

Teachers should check and make sure answer sheets are completely filled in before returning them to the WNYRIC.

5._____           Check to make sure that all bubbles are shaded completely and that all answer sheets are completed properly.

6._____           All “EXTRA ANSWER SHEETS” must have the heading filled out in the shaded area on the front and the student’s name on the back where a line for student’s name has been provided and the student’s 9-digit ID must be filled in at the bottom on each page where the boxes have been provided. (If the student’s ID is not 9 digits, please zero fill preceding the student ID.)

7._____           All “EXTRA ANSWER SHEETS” used must have the L24 form completed.

 

MATERIALS TO BE RETURNED TO WNYRIC

1._____           All used answer sheets.

2._____           All L24’s on top of used “extra” answer sheets and this group on top of all other used answer sheets.  Do not return ALL blank answer sheets.

3._____           This completed assessment checklist.

 

**Student rosters should be returned to your district’s CIO to keep for verification purposes when reports become available.**

 

REMINDER:  PLEASE RETURN THIS COMPLETED CHECKLIST WHEN RETURNING ANSWER SHEETS FOR SCANNING.
